Introduction:

Atu Gawande is a medical doctor first and a journalist/writer second. His specialty in the medical field is General and Gastrointestinal Surgery. He is known for as an expert on reducing error, improving safety and increasing efficiency in surgery. He wrote The Checklist Manifesto to make an impact to the world in terms of preventing unnecessary loss of lives due to human error and carelessness of procedures in the operating room, hospital and or any workplace. He explained in clear examples how a simple checklist saved lives in his own operating room and later observed others do the same

What do you think about the book?

We’ve seen “checklist in action” in so many scenarios in our career and personal life. Anywhere we go there is a checklist present. Examples such as going to the doctor’s office, bringing our car to the dealer for a regular maintenance, doing grocery shopping, planning a trip or a vacation, doing big projects at work, performing a failure analysis, performing a complex repair of a complex system, going to college, joining the military. We’ve seen them all but how often do we study our checklist and revise them so as to yield an efficient flow of execution and how often do we question someone else’s checklist or procedure. At the basic concept of human behavior, we normally just accept what is given to us. No questions ask. “Ok, I’ll fill it up” or “I’ll do it without complaining”. Reasons for that is because most of the time, we do not have control of the checklist or the procedures. I recently finished reading another book about “Managing Chaos and Complexity” and I’ve learned that chaos will always be there because life is a complex system. Humans will always face chaos no matter what. After reading “The Checklist Manifesto” I had another answer on how to manage “chaos” – use a checklist. Simplify every complex task into a one page checklist.

Conclusion:

I did not realize that I had been using a checklist all my life. I have journals, small notebook for personal stuff, small notebook for work related materials, I have action items [actionable items] even the moment I wake up I did not even know it that I am following a checklist. When I write, the tone of my writing is in a checklist form. In some ways, most of us are using a checklist and we don’t even know it. Even for those that don’t even write it on paper- those items on our head when we make a decision, those are mental checklist. Reading The Checklist Manifesto reiterates the importance of a checklist. As mentioned in the book, there are key ingredients of a good checklist and it has to be followed. Silly, but the author made a checklist on “how to make a checklist” because like any other activities, when we function robotically, we miss things. Checklist has to be analyzed several times because there are times we miss a step or the step is confusing. .

Recommendations:
Read the book, it will change the way you look at procedures.
Apply it, it will change the way you create a checklist and procedures.
Live it, it will make your life/work less chaotic.

Verses like this one today put courage and excitement to my dilemma of being a cardiac patient.  It’s been 755 days since my stent#2 and I’m still waiting and expecting that the #3 will come and/or there will be no more #3 and my life will come to an end.  Any day now…. and that is how I live my life.  Everyday I wake up with the intention of living a full day as if this is the last one. Yes, I can bike 40 miles and swim 2.4 miles without any struggle but the risk of my stent#3 will always be there in the corner of my eyes.  With this journal, I can honestly tell you that I am not afraid to be absent from this body because I know that when the time comes, it is not the end of life.  “There is more…” is the promise.  There is that expectation of how I will be going to enjoy the presence of god forever when I leave this earth. How exciting!

FIX: Dell Latitude E6400 Locking Up

PROBLEM: Dell E6400 notebook locking up when viewing Netflix, YouTube, CNN, Timewarner, HBOGO video.
FIX: Cleaned CPU Fan and exhaust with a vacuum cleaner and downgrade the BIOS from version A30 to A19.

The problem happens after the notebook is showing the video after ~10 minutes. The E6400 is connected to an E-Port Plus docking station with dual DVI monitor connection.  The CPU goes to 100% and the notebook becomes inoperable.  The only remedy is to reboot.

I had been looking for answers on how to fix the lock up issue on my main computer at home.  I am using a Dell E6400 Latitude notebook as my main desktop.  The E6400 is docked to a E-Port Plus docking station which provided a dual DVI port, an extra eSATA port, speaker out and MIC in jack, network connection, a parallel and serial port for my  packet radio station.  There are so many reasons why I had not ditched this notebook for something else.  I had to find the fix. My only solution – don’t watch video from Netflix, YouTube, CNN, Timewarner and HBOGO while the computer is docked in the E-Port Plus docking station and I was happy and contented, I was more productive at home when working on a project.  Later on, I bought a E4200 ultra portable notebook which became my video viewing laptop beside my dual monitor.

Today, I was not really looking for a fix on the lock up issue but something else.  The hard drive on my E6400 crashed after I ran an Acronis disk to disk copy to a spare drive.  It was just a regular “out of the blue” activity I had in mind and executed it just for the sake of doing it.  I did not do anything out of the ordinary and had been doing this procedure many times with great success.  After so many hours of trying to understand about how a good running hard drive becomes useless, I came across the popular forum regarding the software lock up issues.  I provided a few links below for your own knowledge and understanding.  Read the articles if you are interested on the root cause of the software lock up problems on E6400.  I don’t want to add more information to what is already been mentioned in the article and forum.  Here’s my $0.02 for the topic:

Causes of the software lock up:
1. Buggy BIOS, I had A30, [downgrading to A19 fixed it.]
2. Intel Speedstep algorithm not optimized [this is a good feature for a notebook to save power but not good for machine used as a desktop]
3. Poor CPU ventilation due to dust accumulation in the fan and exhaust.  The dust changes the cooling system function and ruined the Speedstep process.  When the CPU fan was unable to cool down the CPU temperature , the Speedstep algorithm kicks in to protect the CPU and lower the frequency and voltage.  This is not optimized on E6400 BIOS and it is over compensating/over reacting.

If you are a little bit lazy reading technical stuff and just want to fix the problem, please follow this procedure:
1. Turn off power
2. Get a vacuum cleaner and lint duster [in a can]
3. Blow the exhaust vent and fan with lint duster while the vacuum cleaner is running.
4. Downgrade/Upgrade BIOS to A19

System Information of E6400, I downgraded the BIOS from A30 to A19. The latest and the greatest principle does not work here. A19 BIOS specifically says:

“1.  Updated thermal table.
2.  Removed HDD Acoustic support.
3.  Enhanced INT 19h support.”

The Importance of Checklist

I’ve completed reading The Checklist Manifesto today and my whole perspective of problem solving and dealing with difficult and challenging tasks has changed.  When you get a chance, read the book yourself to gain insights of what a simple checklist can do for you.  I am familiar with a few topics that the author mentioned in the book and I just did not have a profound example of what a real checklist can do in a dire situation.  Reading the book was still a value added activity this week and I wish more topics like this one comes my way.  Knowledge is power, knowledge about the importance of writing a good checklist and doing it are two different things. It is time to take action – create and simplify a checklist.   I have several ideas popping in my mind already about a particular checklist for work related tasks.  Yes, we have Standard Operating Procedures (SOP) in place at work but having a simplified checklist in a one-page document I think is far more powerful.  Of course training is required for each individual handling the situation but having a checklist at hand is a great advantage. I need to create a checklist on how to do a checklist for work and home. ;-0

I’ve provided a video link about the investigation/reenactment of Flight 1549 crash landing at Hudson river.  The event showed that executing a well written checklist can save lives.  It’s a long video but be patient and observe how the pilot and the co-pilot handle the deadly event which resulted to zero fatality.

Brent completed a risky driving task today – he drove for at least two hours in various road condition and environment.  The finale of the driving session today was a 12-mile driving in IH35 [interstate highway 35]. It was a little scary at first but as he began to follow the flow of traffic on a Sunday afternoon commute, he feels like he has the skills to be on the road already.  He committed a few minor mistakes but nothing life threatening.  Todays mileage is roughly 80 miles. Kudos to the $9.00 “Student Driver” magnetic sign that I purchased for this training.   The sign provided a simple “STAY AWAY” message when in close proximity driving on a busy road, drivers are not tailgating us any longer.

Presenting + Communicating 
At work, we are encouraged to take classes to improve ourselves and one of those classes is the “Presentation Class”. Little did I know that this is a class about public speaking [speech class, sort of] where you come up with random idea and present it to the class. Well, I just found my weakness, I am not good in transferring what my brain is thinking into words. The last segment of the class is a ‘final presentation’ (personal or work related) and present to everybody. I’m good in structure and organization and have a lot of ideas to share. We were taught in the class to write what we want to talk about in a 4×6 index card and use mind mapping to organize what we want to deliver in front of the class. On paper, my presentation was rock solid and filled with structured information. We were given 3 minutes to present in front of the class and when it was my turn, it was a disaster – I could not remember anything that I wrote in my index card. All I know that I had an objective – to present about “Shifting Focus to Warning Signs” (more on this later) after a little scare in the middle of the presentation,I regrouped and I was able to deliver the information without any visual/memory aid. At the end of the presentation, the class evaluated me and made comments and I was surprised that there was not much negative comment about the way I presented the message. Not even one person noticed that I made up all the stuff that I was talking about. Most of the negative comments were about gestures/movement. Positive comments from the instructor were the following:
-love your smile
-facial expression
-great use of voice
-inflection volume
-structure
-eye contact

I am a problem solver and a self-made technical writer and I spend most of my time reading books and analyzing problems, writing technical documentations, failure analysis reports and writing this journal everyday. I love to read electrical/electronic schematics, I love flow charts and I love structuring ideas and put everything in perspective. Speaking to people [or to a person] is not in my blood and If you are working with me, get comfortable with my silence because it will be with us all day. I learned today that speaking is a process – that one has to have the idea in mind to be able to fully deliver the message to the audience. Sound so simple but yet most of us open our mouth without really thinking about delivering a great message. Like anything else, one has to practice speaking to people to be able to communicate clearly and effectively.

So from now on, I will be communicating to people verbally. No more Blind Caveman habits of silence.

Windows Server 2012

Microsoft Technet has been talking more about Windows Server 2012 recently and last  night,  I downloaded the .vhd version of Windows Server 2012 hoping that I can get it to work on “boot to VHD” using my spare Dell Poweredge 2850 server which has 8GB of memory and lots of drive space. [PE2850 cannot run Hyper-V].  After a few hours of trying, I’ve learned that the .vhd download version works only with hyper-v.  My main objective is to run w2012 without installing Hyper-V.  My Hyper-V server memory of 8GB is running at the top limit.  It is currently running Exchange 2010/Domain and a Sharepoint 2010/SQL Server 2008.

I have not explored the OS yet nor have I started reading any documentations.  But for those of you who are interested of how the new OS look, here’s some screenshots.

FIX: DVR Scientific Atlanta 8300HD No HDMI Output

Problem: Scientific Atlanta 8300HD DVR Box No HDMI Output
Background:
2 months ago, I’ve seen this problem when I transferred my 8300HD box from the family room to my daughter’s bedroom.  The old DVR connection of my Samsung DLP TV was only a component connection. TV is a bit old and had no HDMI input.  My daughter’s new HDTV had 3 HDMI and so why not connect with HDMI instead of a component connection. I tried … but failed.  I spent hours trying to look for answers and could not find one and even google did not yield any solution and so I told my daughter that she don’t really need an HDMI connection.

A week ago, Tour de France 2012 started and I needed to record some good TDF stages for later viewing but then I discovered that the hard drive of the DVR in my master’s bedroom started making a “clicking noise” and the DVR app warned that I cannot record anymore shows.  I called Time Warner tech support and told me that I needed to swap the box. I had two choices, wait for a week for the technician to come by or go to the nearest TWC office and swap the box.  I choose to DIY the swap and that is where the fun begins. The DVR box that I picked up, the HDMI output is dead-no video output but when I tried the component video connection, it works.  I borrowed my daughter’s DVR box and the same symptom-no HDMI output.  I had two DVR boxes in the house and none works with HDMI and I was challenged/determined to solve the problem…. I did not give up too easily and started looking at serial numbers at the back of the two boxes that has no HDMI output.  I saw no pattern of numbers but when I checked on my son’s DVR box, I saw a different style box.  The box looks the same [front side] but the back connection and tags were not the same.

Long story short, I went back to the local TWC office and requested another swap but this time I requested the boxes with “Made By Cisco” label.  The lady at the TWC office tried to convince me that all boxes work with the newer TV’s but I insisted and she gave me what I requested.  I went home, plug all the connection and 15 minutes later, all HMDI output working. Case closed.

Later I found that Scientific Atlanta was acquired by Cisco in 2005 [view article].

I had an extra day off today due to 4th of July holiday.  Britta arrived in Cebu safely although I have not talked to her yet.  I know she’s been taking cared of…. I  just know.  She’s is Cebu for the funeral services of Lola Bebang and Tia Celia. Tia Celia died a few days after Lola [her mother] passed away.  She just arrived from New York to attend the funeral service a few days earlier.  This few weeks has been tragic around my circle lately.  A week ago, a co-worker of mine died inside the Samsung factory.   Death sometimes is like a reminder that life is short and must be experienced at full 100% engagement.

Britta will be away for a few days.  Running the family by myself is not a big deal anymore since the kids are older now and besides Brittney is already driving and can basically function as a good resource for errands if needed.

I have time to write something tonight.  I am inspired to express what’s in my mind right now for some unknown reasons.  While I was reading “Systems Thinking” late this afternoon, I was looking for a new song to listen on my Spotify account.  I came across Lydia Stanley’s song “Sing Hallelujah" and put the song in repeat mode.  There was something in the song that touched my heart [+few tears].  I know something is up so I had to stop reading and I have to research who this Lydia is and her background.  Fast forward a few minutes of web searching, I came across this youtube video of a lady who was unable to walk for 23 years but then suddenly received her miracle and began to walk by herself.  As she started to take her first step, a thought came to me – “Lord, I want to see a miracle like that in my two eyes”.  With the music still in repeat mode and as I was is deep worship, I closed my eyes and then all of a sudden,  flashes of my past miracles came to me like a replayed video tape and I saw the following in detail:
1. My bike accident that almost killed me in 1989
2. When I fell 15ft from the ceiling at General Milling
3. Ormoc flood in 1991 [5000+ dead]
4. My transition job from the US Army to AMD then to Samsung
5. My myocardial infarction in 2008
6. My being able to bike and swim long distance after my heart attack in 2008.

Then, a still small voice whispered  “no need to see a miracle”, you’ve already experienced it…  many times..

I replied, “I know”.
