Bike clamp came loose while driving. 
I’ve converted this 4 bike rack into 2 and cut off the extra mount. 

FMEA: Failure Mode and Effect Analysis

Failure Mode:
Bike Fell Off From Bike Rack

Cause of Failure:
1. Not enough clamp holding tension. 
2. I did not over-tightened the clamp screw because this is a carbon fiber frame. 
3. This is the first time I’ve used the bike rack since the modification.

Recommended Action:
1. Mount the bike in the inner rack [not the outer]
2. Tighten the clamp at max level
3. Tie a rope for redundancy

Damage to the bike:
1. Scratches in the Ultegra shifter
2. Paint scratches in the top tube where the bike was mounted
3. Pedal deep scratch and slightly bent
4. Drive crankset feels like it is misaligned [but this could be a placebo effect since I saw the pedal has scratches]

Repair Actions:
1. Conducted a comprehensive bike/frame inspection, no functional and structural damaged. ok.
2. Conducted a spoke tension verification on both wheels, ok
3. Tightened all screws, ok
4. Inspect wheel alignment and true, ok|
5. Replace right Look pedal with a spare, ok
6. Ride 2 minutes in the trainer, ok
7. Ride 35 miles outdoor the next day, ok

What did not happened?
1. I did not dropped the bike in the middle of the Interstate Highway 35.
2. I did not cause any accident of the vehicles following me.
3. My bike was not ran over by a speeding cars.

Truly grateful I was looking back at my  rearview mirror when I saw that something was not right.  A quick decision and a decisive response to exit to the side of the highway made a big difference.  The bike fell off as I moved to the right side of the road.  The grassy shallow ditch was also a big help in preventing more damage to the bike.
